Voluntary Participation
Spiritual Care Not Professional Counseling spiritual in nature, rooted in biblical prayer and Christian faith. It is not medical, psychological, psychiatric, or professional counseling, and no guarantees or promises regarding outcomes are made. Ministry team members are not licensed counselors, therapists, psychologists, or medical professionals. If you require medical or mental health care, you are responsible for seeking those services independently.
Personal Responsibility & Participation You may be encouraged to engage in prayer, repentance, forgiveness, Scripture reading, accountability, or other spiritual practices as part of the ministry. Your participation is voluntary, and you remain responsible for your own spiritual, emotional, and physical well-being.
Confidentiality Information shared during ministry will be treated with care and confidentiality within the ministry team. However, confidentiality may be limited if there is concern for imminent harm to yourself or others, disclosure of abuse or neglect of a minor or vulnerable person, or if disclosure is otherwise required by law. In such cases, information may be shared with appropriate church leadership or civil authorities as necessary.
Audio Recording for Ministry Notes With your consent, your ministry appointment may be audio recorded for the limited purpose of assisting the ministry team with accurate recall and transcription of confidential internal ministry notes and to support continuity in follow-up care. Audio recordings and any resulting transcripts are not shared publicly or outside the ministry team. You may request access to your own transcript or notes derived from the recording, and you may withdraw consent for audio recording at any time before or during the appointment.
Release and Waiver of Liability By agreeing below, you acknowledge that you have read and understand this consent and waiver, that you have had the opportunity to ask questions, and that you voluntarily consent to receive prayer, deliverance, and inner healing ministry. You release and hold harmless the church, its pastors, leaders, employees, volunteers, and ministry team members from liability for emotional, spiritual, or other responses that may occur as a result of this ministry, to the fullest extent permitted by Colorado law.
